At Chic Plastic Surgery, we understand that unexpected circumstances may arise. Our goal is to provide every patient with flexibility whenever possible while maintaining fairness to our surgeons, operating room staff, and other scheduled patients.

Initial Evaluation Fee

The Initial Evaluation Fee is required to determine whether a patient is a candidate for surgery.

The Initial Evaluation Fee is earned upon receipt, is applied toward the total cost of the selected procedure, and is non-refundable, regardless of whether the patient decides not to proceed with surgery or is determined not to be a surgical candidate.

Cancellation Policy

If you wish to cancel your procedure, you must submit a written cancellation request.

• Cancellation requests received within fourteen (14) days after signing the Surgical Agreement may be eligible for a refund in accordance with the terms of the Surgical Agreement. The Initial Evaluation Fee and any other non-refundable fees are excluded from any refund.
• Cancellation requests received after the fourteen (14) day cancellation period are not eligible for a refund except as otherwise determined by Chic Plastic Surgery in its sole discretion or as expressly provided in the Surgical Agreement.

Rescheduling

Patients may request to reschedule their procedure by submitting a written request.

• Requests received 30 days or more before the scheduled surgery date may be accommodated without a rescheduling fee, subject to surgeon availability.
• Requests received within 30 days of the scheduled surgery date may be subject to rescheduling or procedure change fees as outlined in the Surgical Agreement.

Medical Requirements

Patients are responsible for completing all required laboratory testing, medical clearances, and any additional evaluations requested by their surgeon.

All required laboratory results and medical clearances must be received by our office no later than ten (10) days before the scheduled procedure.

Failure to comply with these requirements may result in postponement or cancellation of the procedure.

Changes in Health

Patients must immediately notify our office of any change in their medical condition, including but not limited to:

• New medical diagnoses or conditions
• Changes in medications
• Pregnancy
• Fever, infection, flu-like symptoms, COVID-19, or other illnesses
• Hospitalizations or emergency medical treatment
• Any other condition that may affect the safety of surgery

Failure to disclose medical information or changes in health may result in postponement or cancellation of the procedure and may affect refund eligibility under the Surgical Agreement.

Drug, Nicotine & Alcohol Testing

Patients must comply with all surgeon requirements regarding nicotine, alcohol, prescription medications, recreational drugs, and other substances.

A positive test for any substance that, in the surgeon's medical judgment, makes surgery unsafe may result in postponement or cancellation of the procedure.

Surgery Eligibility

Each surgeon establishes the medical criteria that patients must satisfy before surgery, including, but not limited to, BMI requirements, laboratory results, medical clearances, nicotine-free status, pregnancy status, and overall medical fitness.

Failure to satisfy these requirements may result in postponement or cancellation of the procedure.

Any refund request related to surgery eligibility, medical requirements, or cancellation under these circumstances shall be reviewed solely at the discretion of Chic Plastic Surgery and in accordance with the executed Surgical Agreement.

Governing Agreement

This Refund & Cancellation Policy is provided as a general summary for informational purposes only.

In the event of any inconsistency between this policy and the executed Surgical Agreement, the terms of the Surgical Agreement shall govern and control.
